30-Day Visa-Free Entry vs Official S2 Medical Visa Decision Compass

Based on the complexity of the condition, the treatment cycle and the expected length of stay, choose the most reliable lawful entry document and rule out the credit impact that overstaying has on later cross-border referrals.

Suitable for lightweight treatment decisions, short expert face-to-face outpatient reviews and non-invasive investigation plans

check_circleTreatment scenarios that fit best:
  • •In-person specialist clinics with leading Grade 3A physicians (face-to-face MDT multidisciplinary review)
  • •Single high-end imaging investigations (such as China's advanced 7.0T ultra-high-field MRI and new-generation PET-CT)
  • •Gene re-sequencing after resistance to targeted oncology drugs and confirmation of the clinical medication plan
  • •Minimally invasive day-case outpatient treatment for mild conditions, or initial TCM conditioning
warningPractical risk warning:

Strictly limited by a 15 calendar-day countdown. If, in the middle of treatment, the condition proves complex and an urgent major general anaesthesia operation is required, or a post-operative complication or infection occurs, the hospital may face considerable pressure in approving an extension because the patient lacks lawful long-term S2 medical backing.

Guidance on Chinese customs compliance rules for reasonable personal-use medication:

Original box, original packaging and doctor's prescription declaration:

Narcotic and psychotropic controlled medication (such as morphine):

Carry no more than a 7-day supply of strong opioid analgesics; a red prescription diagnosis certificate from a legitimate tertiary hospital is required, and the Customs Declaration Form for Incoming and Outgoing Passengers' Luggage of the People's Republic of China must be completed at Chinese entry customs.

KNOWLEDGE BASE

Frequently Asked Questions on Visas and Entry (FAQ)

Removing uncertainty: every practical question Malaysian patients have about cross-border medical visas is answered clearly here.

How long can a Malaysian passport holder stay in China visa-free per entry?expand_more
Under the current China–Malaysia mutual visa exemption, a Malaysian ordinary passport holder entering China for leisure travel, visiting relatives, business, exchange visits, private matters, medical treatment and similar purposes may enter visa-free for a single stay of up to 30 days, with cumulative stays not exceeding 90 days within the 180 days counted back from the date of entry. Short consultations, reviews and health checks can therefore be arranged entirely visa-free; once the 90-day cumulative ceiling within 180 days is reached, or a single stay needs to exceed 30 days, the case falls on the side that requires a visa.
In which cases must a visa be applied for in advance, and what obligations apply after visa-free entry?expand_more
A visa must be obtained before entry in these cases: a single stay of more than 30 days; cumulative stays that would exceed 90 days within 180 days; and activities requiring prior approval such as settling, working, studying or news reporting — visa-free entry is not available for them. Two obligations also apply once you have entered visa-free. First, accommodation registration: hotels register their guests, but anyone staying somewhere other than a hotel must register with the local public security authority within 24 hours of arrival, either in person or through the host. Second, carry documentation that matches your purpose of entry — a hospital invitation letter, flight and hotel bookings — in case the border inspection authority asks for it. You must leave before the visa-free period expires; if urgent circumstances require you to stay longer, apply to the local public security bureau’s exit-entry administration for a stay permit before it expires.
What is this page’s visa-free information based on, and how often is it verified?expand_more
It is based on the Agreement between the Government of the People’s Republic of China and the Government of Malaysia on Mutual Exemption of Visas for Holders of Ordinary Public-Service Passports and Ordinary Passports (effective 17 July 2025, valid five years and automatically renewed for a further five), together with the Chinese Embassy in Malaysia’s Questions and Answers on the China–Malaysia Mutual Visa Exemption Agreement published on 1 August 2025. YILV CHINA verified this in September 2026. The agreement itself is a long-term arrangement, but port-level implementation rules can still change, so check the latest notice from the Chinese embassy or consulates in Malaysia before travelling; for individual cases we re-check against the current rules at the time of coordination.
Can I enter China visa-free first, and then apply locally to continue my stay once admission is confirmed?expand_more
You can, but it must be done before your visa-free stay expires. Under the guidance of the Chinese Embassy in Malaysia, if you need to remain in China beyond the visa-free period you must apply to the local public security bureau's exit-entry administration for a stay or residence permit procedure before the period ends. In practice this requires a Certificate of Critical Illness Hospitalisation and a Medical Guarantee Official Letter from the receiving Grade 3A public hospital; the official letter is issued by the hospital's foreign affairs office, and family members may take that letter to the municipal public security bureau's exit-entry administration to handle the procedure. To avoid port-of-entry and approval volatility, if surgery or chemoradiotherapy is already known to be required, we recommend completing the S2 medical visa in Malaysia before departure — a single stay beyond 30 days, or repeated long treatment courses, falls within S2 scope.
How many accompanying family members can be applied for at most? Can non-immediate relatives (such as friends or a domestic helper) obtain an S2?expand_more
Under the relevant rules of the Chinese embassy and consulates in Malaysia, the S2 accompanying visa is in principle issued to the patient's immediate family members (spouse, parents, children, parents-in-law, siblings); normally 1 to 3 accompanying family members may be approved for one critical case. If the companion is an unrelated carer or friend, an S2 accompanying visa cannot be applied for on the basis of a notarised kinship certificate; such accompanying persons may instead use the visa-free arrangement for Malaysian citizens directly (up to 30 days per entry), or separately apply for the appropriate visa.
Does a Grade 3A hospital in China charge for the Medical Treatment Invitation Letter? How likely is a visa refusal?expand_more
Chinese Grade 3A public hospitals themselves do not sell official letters or charge any agency fee for them, but issuing the letter must be based on the rigorous clinical admission process of "medical records filed in advance, department chief review and agreement to accept, and reservation of a premium ward". YILV CHINA charges only a transparent service fee for standardised translation of medical records and bilateral specialist liaison and coordination. Provided the Grade 3A hospital gives its stamped support and there is no major criminal record in Malaysia, the refusal rate at the Kuala Lumpur visa center for such humanitarian medical S2 applications is virtually zero.
For Muslim patients, how can the visa stay period be reconciled with Halal food and religious prayer during treatment?expand_more
YILV CHINA provides full Halal-Friendly care support for Malaysian Muslim families: the international departments of the Grade 3A hospitals we coordinate with all allow dedicated Halal meal arrangements (prepared and delivered independently by a Halal restaurant certified by the local Islamic association), and the prayer direction (Qibla) can be clearly marked in the premium ward with prayer mats provided. When applying for the S2 visa, family members can be granted visas together, ensuring the whole treatment cycle both complies with Chinese exit-entry regulations and fully respects Islamic customs.
